Transition Assistance

Transition Assistance are service available to individuals who are transitioning or wish to transition from an institutional setting, such as nursing facility, to a community-based setting.

Money Follows the Person (MFP) Demonstration

Money Follows the Person (MFP) Demonstration Program is a voluntary program that helps elders and adults with disabilities who want to move from facility-based care back to the community. People in the Money Follows the Person Demonstration Program can receive transition coordination, care management, transitional assistance services, orientation and mobility, and specialized adaptive equipment.  MFP is administered by MassHealth. 

Nursing Home Transition

Nursing Home Transition services are provided to help individuals with disabilities transition from a nursing home into community living. Nursing Home Transition services are available through the Massachusetts Rehabilitation Commission.
