Personal Care Services

Personal Care Services are services that provide individuals with functional limitations with assistance with self-care and daily living tasks such as bathing, toileting, ambulation and feeding.

Adult Foster Care (AFC)

Adult Foster Care (AFC) is a program for individuals age 16 and older who require physical assistance or cueing & supervision for 1 or more Activities of Daily Living (ADL), such as eating, dressing, and bathing. AFC members live with a qualified paid caregiver. Services include assistance with personal care, medication, meals, laundry, nursing oversight and care management.AFC is available through MassHealth.

Group Adult Foster Care (GAFC)

Group Adult Foster Care (GAFC) is a program for adults age 22 and older who require physical assistance for 1 or more Activities of Daily Living (ADLs) such as eating, dressing, and bathing. Services are provided in qualified group housing setting.  Services include assistance with personal care, medication, meals, laundry, nursing oversight and care management. GAFC is available through MassHealth.

Personal Care Attendant (PCA) Program

Personal Care Attendant (PCA) Program is a program for individuals of any age who require hands-on physical assistance with 2 or more Activities of Daily Living (ADLs), such as eating, toileting, and transferring. The program is self-directed by the individual or their surrogate.  This means that the individual hires, trains, and manages their PCA worker. The PCA Program is available through MassHealth.
